Join CVHS HEALTH SERVICES as a Pharmacist – Clinical/Operations Hybrid, FQHC Setting in United States.

- will include performing detailed medication reconciliations, comprehensive medication reviews, and in-person medication therapy management reviews for clinic and pharmacy patients to identify patient specific medication therapy problems and help manage drug therapies and interventions to achieve positive patient outcomes.

- or Pharm D. Licensed to practice pharmacy in the Commonwealth of Virginia. License is free of restrictions to bill Medicare and Medicaid.
