Join CVHS HEALTH SERVICES as a Pharmacist – Clinical/Operations Hybrid, FQHC Setting in United States.

Key Responsibilities

will include performing detailed medication reconciliations, comprehensive medication reviews, and in-person medication therapy management reviews for clinic and pharmacy patients to identify patient specific medication therapy problems and help manage drug therapies and interventions to achieve positive patient outcomes.

or Pharm D. Licensed to practice pharmacy in the Commonwealth of Virginia. License is free of restrictions to bill Medicare and Medicaid.
