Community Social Worker

Key Responsibilities

The Community Social Worker I supports the mission by empowering and facilitating clients, who are homeless or at risk with disabling conditions, to achieve independence and stability through supportive services and goal achievement. Key duties include establishing rapport, developing person-centered service plans, providing comprehensive support, and acting as a liaison for housing maintenance and resource access.

Requirements

Candidates need a Bachelor's degree or equivalent combination of education and two to three years of related experience, preferably working with homeless persons or those with mental illness or substance abuse. A valid driver's license, reliable transportation, and strong documentation/critical thinking skills are mandatory.
