What You'll Do
Conduct patient assessments and develop individualized plans of care.
Coordinate care with physicians, patients, families, and other healthcare professionals.
Educate patients and caregivers on care plans, treatments, and ongoing health needs.
Supervise and support LPNs, HHAs, and CNAs to promote quality patient care.
Review and update plans of care as needed.
Maintain accurate, timely, and complete clinical documentation.
Participate in case conferences and quality improvement initiatives.
Help ensure patients receive safe, compassionate, and effective care in their homes.
What We're Looking For
Graduate of an accredited nursing program.
Active Indiana RN license or a qualifying multistate license recognized by Indiana.
Minimum of 2 years of nursing experience; home health experience preferred.
Current CPR certification.
Strong clinical, communication, leadership, and organizational skills.
A caring, patient-centered approach and a genuine desire to make a difference.

Key Responsibilities
The RN Case Manager conducts patient assessments, develops individualized care plans, and coordinates care with physicians and families. They also supervise LPNs, HHAs, and CNAs while maintaining accurate clinical documentation and participating in quality improvement initiatives.
Requirements
Candidates must be graduates of an accredited nursing program with an active Indiana RN license. A minimum of 2 years of nursing experience and current CPR certification are required.
