What You’ll Do
Deliver Patient-Centered Care
Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential
Develop and implement individualized care plans in collaboration with physicians and the care team
Provide hands-on therapy to improve mobility, strength, balance, and independence
Educate patients and caregivers on treatment plans, exercises, and safety strategies
Drive Outcomes & Recovery
Monitor patient progress and adjust treatment plans as needed
Support safe transitions of care and contribute to discharge planning
Identify additional patient needs and collaborate with interdisciplinary teams
Collaborate & Communicate
Partner closely with nurses, therapists, and physicians to ensure coordinated care
Document patient care and progress accurately and timely within the EMR
Participate in case conferences and team meetings
Lead & Support
Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines
Provide guidance and education to caregivers and support staff
What You Bring
Graduate of an accredited Physical Therapy program (DPT or Master’s)
Current PT license in the state of practice
Minimum 1–2 years of experience (home health experience preferred)
Strong clinical assessment and critical thinking skills
Excellent communication and patient education abilities
Current CPR certification
Valid driver’s license, reliable transportation, and auto insurance

Key Responsibilities
The Physical Therapist will evaluate patients, develop individualized care plans, and provide hands-on therapy to improve mobility and independence in a home health setting. They will also collaborate with interdisciplinary teams, document patient progress, and supervise Physical Therapist Assistants.
Requirements
Candidates must hold a degree from an accredited Physical Therapy program and possess a current state PT license. A minimum of 1–2 years of experience, preferably in home health, along with current CPR certification and a valid driver's license are required.
