Key Responsibilities
The Physical Therapist will evaluate patients' functional status and develop individualized care plans to improve mobility, strength, and independence within their homes. They will also collaborate with interdisciplinary teams, document progress in the EMR, and supervise Physical Therapist Assistants.
Requirements
Candidates must hold a degree from an accredited Physical Therapy program and possess a current state PT license. A minimum of 1-2 years of experience, current CPR certification, and a valid driver's license are required.
