Key Responsibilities
The Physical Therapist will evaluate patients' functional status and develop individualized care plans to improve mobility, strength, and independence in a home health setting. They will also collaborate with interdisciplinary teams, document progress in the EMR, and supervise Physical Therapist Assistants.
Requirements
Candidates must hold a degree from an accredited Physical Therapy program and possess a current state license. A minimum of 1-2 years of experience, preferably in home health, along with a valid driver's license and CPR certification is required.
