Key Responsibilities
The Physical Therapist will evaluate patients' functional status and develop individualized care plans to improve mobility, strength, and independence in a home health setting. They will also collaborate with interdisciplinary teams, document patient progress, and supervise Physical Therapist Assistants.
Requirements
Candidates must hold a degree from an accredited Physical Therapy program and possess a current state PT license. A minimum of 1–2 years of experience is required, along with current CPR certification and a valid driver's license.
