Key Responsibilities
The RN Case Manager is responsible for processing palliative care referrals, scheduling patient visits, and coordinating care goals among providers. They also ensure accurate documentation, manage admissions and discharges, and serve as a liaison for the medical team.
Requirements
Candidates must hold a current, active, and unrestricted RN or LPN license in the state of practice. A minimum of three years of nursing experience is required, with preference given to those with a background in hospice, palliative, or home health care.
