Work Schedule

This position offers weekends off. Work with Hospice of the West in Mesa, Arizona.
Key Responsibilities
The RN Case Manager will provide hands-on hospice nursing care and coordinate services for patients and families within the community. They will also collaborate with the clinical team to develop personalized care plans and participate in on-call rotations.
Requirements
Candidates must hold a current, unencumbered Registered Nurse license and have at least one year of experience in a medical-surgical or acute care setting. Proficiency with EMR systems, a valid driver's license, and current CPR certification are also required.
