As a Nurse Practitioner (NP) or Physician Assistant (PA) at Conviva Senior Primary Care, you will be part of a collaborative, multidisciplinary team that includes physicians, pharmacists, care coaches, behavioral health specialists, and more—all working together to deliver personalized, high-quality care.
In this role, you will work onsite at our Jourdanton clinic. You will report to our Associate Medical Director.

Your Role & Responsibilities
You will provide comprehensive care to seniors following clinical standards.
You will collaborate daily with your care team to ensure coordinated, high-quality care.
You will manage referrals, hospital/SNF coordination, DME, and home health services.
You will document care accurately and efficiently with support from quality-based coders.
You will participate in on-call rotation and support growth plans at your center.
You will have passion for providing value-based care.
What We're Looking For
Required:
You will have a Master's Degree in Nursing or completion of a PA program with board certification.
You will have an active, unrestricted NP or PA license in the state of practice.
If selected for this role, you will be required to be screened for TB
Preferred:
You will have 2 or more years of experience in value-based care, primary care or Medicare Advantage settings.
You will have an active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and/or Medicaid and Medicare numbers

Pay Range
The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$123,800 - $170,300 per year

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.

Key Responsibilities

Provide comprehensive, high-quality primary care to seniors within a collaborative, multidisciplinary team. Manage patient referrals, hospital coordination, and documentation while participating in on-call rotations.

Requirements

Requires a Master's Degree in Nursing or PA program completion with board certification and an active, unrestricted license in the state of practice. Preference is given to candidates with 2+ years of experience in value-based care or Medicare Advantage settings.
