Key Responsibilities
The Social Worker provides psychosocial support to residents and families while coordinating care plan meetings and facilitating referrals to community resources. They are responsible for completing MDS assessments, discharge planning, and documenting interactions in accordance with state and federal standards.
Requirements
Candidates must hold a bachelor's or master's degree from an approved Social Work program and be licensed by the Kansas Board of Behavioral Sciences. Prior experience in a similar environment is preferred, along with the ability to maintain professional standards and adhere to safety and infection control policies.
