Key Responsibilities

They act as a liaison between patients, families, and healthcare teams to ensure continuity of care and facilitate access to essential services.
Requirements
Candidates must hold a current Montana RN license or a multi-state compact license and possess strong communication and organizational skills. Experience in managing chronic conditions and knowledge of electronic health records are highly preferred.
