Key Responsibilities
The physical therapist will provide evaluative, consultative, and treatment interventions to home health patients in their place of residence. They are responsible for developing and managing patient care plans while collaborating with an interdisciplinary team to ensure optimal functional outcomes.
Requirements
Candidates must hold a valid professional physical therapy license in the state of practice and be a graduate of an accredited physical therapy program. Prior home health experience is preferred.
