Key Responsibilities

Provide evaluative, consultative, and treatment interventions to home health patients in their place of residence. Collaborate with the interdisciplinary team and physicians to develop, integrate, and manage patient care plans.

Requirements

Must hold a valid professional license in the state of practice and be a graduate of an accredited Physical Therapy program. Prior home health experience is preferred.
