Job Description

Collage Home and Community Rehabilitation Partners is a growing organization dedicated to excellence, honesty, and ethics in our treatment of our clients and each other. We provide treatment for traumatic brain injuries, spinal cord injuries, CVAs, and other neuro or complex ortho diagnoses in the most functional setting there is - a person's own home and community.
We are looking for experienced Physical Therapists who want to provide truly patient-centered care in Atlanta, GA, and surrounding areas - with PRN positions available. Our interdisciplinary teams create integrated treatment plans that are practiced in patients' homes and communities. Our therapists are creative in their approach and incorporate a variety of activities to ignite interest while addressing functional goals. This evidence-based approach results in durable outcomes.

Areas of responsibility include but are not limited to:
* Evaluation and treatment of impairments including lower extremity function, mobility, vestibular skills, adaptive equipment needs, environmental safety needs
Qualifications
* Completion of Bachelor's degree in Physical Therapy and state licensure as well as experience in the treatment of neurologically related disorders
* Excellent planning, organization, writing, and time management skills are required
* Ability to communicate with families, professionals, and co-workers is a must
