Job Description
Kaniksu Community Health is a non-profit, award winning Community Health Center that provides integrated, team-based healthcare in north Idaho. - Dispenses medications prescribed by physicians and other health practitioners, which includes providing proper counseling to patients.
- Communicates individualized care plans effectively with patients and healthcare providers.
- Provides patient care services such as medication therapy management and disease management.
- Conducts wellness and preventative health programs.
- Ensures compliance with all relevant laws of the applicable State Board of Pharmacy.
- Administration of immunizations as allowed by State Board of Pharmacy.
- Provides patient-centered care that will improve clinical outcomes and reduce total cost of care.
- Supports patients in their initiation and maintenance of new therapy; providing education, optimizing utilization and establishing adherence.
- Identify and take specific corrective action for drug-induced problems.
- Monitor patients for appropriateness and response to therapy.
- Order and analyze laboratory and diagnostic test data to modify drug therapy and dosing as necessary.
- Proper documentation in patient’s EHR.
- Engages patients at clinically relevant times to deliver impactful clinical pharmacy services.
- Maintains department performance levels for quality and productivity.
- Identifies opportunities for the development of new clinical programs; takes an active role in the development and implementation of clinical projects.
- Performs other projects, work, and activities as assigned to ensure continuous service to our patients.
- Minimum Bachelor's Degree in Pharmacy required.
- Current Idaho State Board of Pharmacy license in good standing.
- Certified immunizer.
- Experience with the 340B program highly beneficial.
- Experience in an ambulatory or clinical setting highly beneficial.
- 1+ years of Retail pharmacist experience desired.
- 1+ years of CHC pharmacy experience desired.
- Experience in a high volume pharmacy beneficial.
- Ability to maintain strict corporate confidentiality and HIPAA compliance.

Key Responsibilities
The pharmacist will dispense medications, provide patient counseling, and manage individualized care plans within a clinical setting. They are also responsible for administering immunizations, monitoring therapy responses, and contributing to the development of clinical programs.
Requirements
Candidates must hold a minimum of a Bachelor's degree in Pharmacy and a current Idaho State Board of Pharmacy license. Experience in an ambulatory or clinical setting and certification as an immunizer are highly beneficial.
