Provide chronic care management and chronic non-cancer pain management through independent patient visits and collaborative care, including participation in medication therapy management programs such as diabetes management, tobacco cessation, and anticoagulation services.
Manage medication refills for established patients as the primary point of contact within the care team, unless otherwise directed by the provider.
Support providers and the interdisciplinary care team with clinical consultation within scope of practice, including laboratory interpretation, non-urgent medical questions, and prior authorization reviews.
Counsel and educate patients on the safe and appropriate use of medications and medical devices during in-person or approved off-site encounters.
Develop, deliver, and participate in medication and drug education programs for residents, providers, and clinical team members.
Maintain current knowledge of medications, treatment guidelines, and disease states to support clinically appropriate prescribing protocols, prior authorization decisions, and evidence-based medication use.
Contribute to the development and implementation of medication management protocols, clinical review criteria, and formulary recommendations based on best practices, clinical efficacy, cost-effectiveness, contract availability, and third-party billing considerations; communicate recommendations to clinical leadership.
Document patient encounters, clinical recommendations, and related activities accurately and promptly in the electronic health record (EHR).
Participate in peer review of fellow Clinical Pharmacists in accordance with the applicable Collaborative Drug Therapy Agreement.
