Key Responsibilities
Provide skilled physical therapy services in patient homes as ordered by a physician, including assessments, treatment planning, and patient education. Maintain accurate clinical documentation and collaborate with the interdisciplinary team to ensure quality care and positive recovery outcomes.
Requirements
Must hold a current Physical Therapist license in the state of employment and have at least one year of experience in an acute setting. A valid driver's license and auto insurance are required for travel to patient locations.
