Key Responsibilities
The Physical Therapist provides skilled home health services, including performing OASIS assessments and developing individualized plans of care. They are responsible for treating patients to restore function, educating families, and maintaining accurate clinical documentation in compliance with Medicare guidelines.
Requirements
Candidates must hold a current Physical Therapist license in the state of employment and have at least one year of experience in an acute setting. A valid driver's license and auto insurance are required, with home care experience being highly desired.
