RN, Case Manager

Key Responsibilities

The RN Case Manager assesses patient needs and coordinates the Plan of Care with the Interdisciplinary Team to provide clinical and palliative support. They conduct home visits to ensure patient safety, medication compliance, and continuity of care within the community.

Requirements

Candidates must hold a current RN license and possess at least one year of nursing experience in hospice or hospital settings. A valid driver's license, reliable transportation, and active BLS certification are required for this role.
