Key Responsibilities
The Advanced Care Partner collaborates closely with approximately five Primary Care Physicians (PCPs) to share the care and continuity management of their patient panels, focusing on maintaining task category Service Level Agreements (SLAs). Responsibilities include answering clinical questions, making clinical decisions based on incoming data, managing chronic condition medications, providing necessary visits, and completing clinical paperwork.
Requirements
Candidates must have 5+ years of experience as an NP or PA, deep proficiency in remote/virtual medical care, and expertise in chronic disease management and high-risk caseloads, requiring current licensure in California. Strong self-starting discipline, high technology proficiency, and excellent collaborative communication skills are also essential.
