Key Responsibilities
The social worker facilitates referral coordination and provides psychosocial support to hospice patients and their families within a hospital setting. They collaborate with the interdisciplinary team to develop care plans, conduct assessments, and assist with safe discharge planning.
Requirements
A Master of Social Work (MSW) degree is required for this position. Candidates must also hold or be eligible to obtain an LSW or LCSW license within two years of hire.
