Key Responsibilities
Collaborate with interdisciplinary teams and hospitals to develop comfort-focused care plans and facilitate safe patient transitions. Conduct psychosocial assessments and perform home, hospital, and facility visits to support patients and families.
Requirements
Requires a Master of Social Work (MSW) degree and an LSW or LCSW license within two years of hire. Two years of experience in a healthcare setting is preferred.
