Key Responsibilities
The Field Remote Nurse Practitioner/Physician Assistant conducts in-home assessments and provides medical and social support to patients in the Care Anywhere program. They also partner with the medical team to develop care plans and coordinate care with specialists and primary care providers.
Requirements
Candidates must hold a Master's degree from an accredited NP or PA program and possess an active California license. Required certifications include NPI, DEA, and BLS, along with a valid driver's license and automobile insurance.
