Palliative Nurse Practitioner (58618)

Key Responsibilities

The Nurse Practitioner will conduct comprehensive palliative care visits, including assessment, diagnosis, and symptom management for patients with serious illnesses. They will also develop individualized care plans in collaboration with an interdisciplinary team and coordinate care for hospice patients.

Requirements

Candidates must hold an active Nurse Practitioner license in Wisconsin and possess strong clinical assessment and documentation skills. While experience is preferred, the employer is willing to train new Nurse Practitioners.
