Job Description
We're needing a Primary Care Physician practicing in Skilled Nursing, Assisted Living and Independent Living facilities for a Provider run group in Central Florida. We are a large practice with over 25 practitioners that serve the skilled nursing facilities, assisted living facilities and our homebound patients in the comfort of their home in 14 counties. Key Responsibilities
Provide primary care services to geriatric patients within skilled nursing, assisted living, and independent living facilities. Focus on delivering excellent patient care while actively working to reduce hospital readmissions.
Requirements
Must be a qualified Physician capable of practicing primary care for the geriatric population. Ability to travel to various facilities and homebound patients across 14 counties in Central Florida is required.
