Job Description

Visiting Nurse Service Westchester, established in 1901, is looking for a Physical Therapist to be based in Putnam or Dutchess counties.

Salary Range: $100,000 - $116,000 / year

Physical Therapist Position Description

Our physical therapists assess patients for functional limitations and develops a therapy plan of treatment with specific goals in collaboration with the patient, family, nurse, physician and other health team members. They provide direct care to treat the medical/surgical condition through the use of accepted physical therapy modalities.

The Physical Therapist also Instructs and supervises the patient/family and home health aide (and others) in the home exercise program that they write and other aspects of patient care as appropriate and documents their response to instruction.

Continually re-evaluates the patient’s condition and modifies the plan of treatment as appropriate.

Our PT identifies and reports significant clinical information to, physician, clinical manager, primary nurse coordinating services and other disciplines. They document all clinical evaluations, treatments and patients response to therapy.

Qualifications

  • NYS professional License and current registration as a Physical Therapist.
  • One (1) year of full time experience in direct provision of physical therapy service.
  • Ability to assess and respond to the needs of patients and families in the home setting.
  • Home care experience preferred.
  • Ability to utilize industry technology such as a tablet when in the field with patients.
  • Experience with Oasis assessments is preferred.
  • Ability to drive with automobile in good working condition and available for use in the field.
