Make a lasting difference in the lives of older adults

Join ElderOne, Rochester Regional Health’s Program of All-Inclusive Care for the Elderly (PACE), at North Park. ElderOne helps older adults continue living in their own homes and communities by bringing medical, social, and supportive services together around each participant’s needs.

Why Join ElderOne at North Park?

Collaborative care: Work with an interdisciplinary team to assess participants’ needs, develop care plans, and coordinate services.

Meaningful patient relationships: Get to know participants and their families while caring for a focused population.

Time for patient care: Practice without the usual demands of billing, insurance administration, or productivity-based pressures.

Whole-person support: Collaborate with a team that addresses medical, behavioral health, social, and functional needs.

A community-based practice: Work at North Park and support older adults in the surrounding community.

ElderOne operates within the PACE model, which coordinates and provides comprehensive care for eligible participants through Medicare and Medicaid.
