About the Role
Skilled Nursing Pharmacy in Hayward, CA is looking for a dedicated PM Pharmacist to join our team on a per diem basis. This is a fantastic opportunity for a licensed pharmacist to make a meaningful impact by supporting long-term care patients with expert medication management during evening hours. Responsibilities
Dispense and verify medications accurately for skilled nursing facility patients during PM shift hours Review and process medication orders, ensuring compliance with state and federal regulations Perform drug utilization reviews (DUR) and monitor for drug interactions, contraindications, and allergies Collaborate with nursing staff and healthcare providers to resolve medication-related issues Ensure proper documentation and recordkeeping in pharmacy management systems Oversee pharmacy technicians and support staff during shift operations Maintain controlled substance compliance and inventory management protocols Requirements: Active California Pharmacist License (RPh) in good standing Experience in long-term care, skilled nursing, or institutional pharmacy preferred Familiarity with pharmacy management software and electronic medication records Strong knowledge of federal and California pharmacy regulations for skilled nursing facilities Ability to work independently and make sound clinical decisions during PM hours Excellent communication and interpersonal skills for cross-functional collaboration Flexibility and reliability as a per diem team member About Us: Skilled Nursing Pharmacy is a trusted provider of long-term care pharmacy services dedicated to improving the quality of life for residents in skilled nursing facilities across the Hayward, CA area.
