Key Responsibilities
Provide primary care to a panel of seniors with complex chronic conditions in an outpatient clinic setting. Collaborate with a dedicated care team to ensure high-quality, value-based patient outcomes.
Requirements
Must hold an MD or DO degree with active, unrestricted medical licensure and DEA registration. Requires at least 2 years of outpatient experience and board certification or eligibility in Family or Internal Medicine.
