Job Description
Guaranteed full-time opportunity at CareCo Medical Home Health, a Medicare-certified home health agency.
Responsibilities
Assess, evaluate, and provide care for patients in a home care setting.
Observe, record, and coordinate care with multi-disciplines regarding the patient's treatment and any change in condition
Teach supervise, and counsel the patient and family regarding home therapy procedures and other care needs as appropriate to the patient’s condition
Qualifications
Current license as a Physical Therapist in Connecticut
Valid driver’s license
Reliable transportation and willing to travel in a designated area that may include New London, Windham, and Middlesex counties
Strong communication and interpersonal skills
Home health experience including Medicare preferred
