Key Responsibilities

  • provide quality patient care.
  • develop and manage the interdisciplinary patient discharge plan.

Requirements

  • This position is accountable for the quality of clinical services delivered by both them and others and identifies/resolves barriers which may hinder effective patient care.
  • Manages individual patients across the health care continuum to achieve the optimal clinical, financial, operational, and satisfaction outcomes.
  • Provides assessment, direct psychoeducation, and direct psychotherapy to individuals, families, and groups.
  • Coordinates all aspects of patient care from admission to discharge, interacting with the physician and other members of the clinical treatment team to provide quality patient care.
