Behavioral Health Case Manager

Key Responsibilities

This position provides comprehensive care coordination by assessing the patient’s plan of care, developing, implementing, monitoring, and documenting resource utilization and patient progress through care. Responsibilities include managing individual patients across the continuum to achieve optimal outcomes, providing direct psychoeducation and psychotherapy, and coordinating all aspects of patient care from admission to discharge.

Requirements

Candidates must possess a Master's degree in social work or a related field, or equivalent experience, along with typically five years of clinical experience and required state licensure (social work, professional counseling, or marriage and family). Specific requirements include a working knowledge of care management, hospital/community resources, and the ability to work effectively in an interdisciplinary team.
