The case manager will conduct psychosocial assessments and implement treatment and discharge plans in collaboration with an interdisciplinary team. They will also provide crisis intervention and supportive therapeutic interactions while maintaining accurate clinical documentation.
Candidates must hold a Master's degree in social work or a recognized mental health field and be eligible for or possess relevant licensure such as LSW, LPC, or LMFT. One year of experience in a behavioral health environment is preferred, along with CPR/BLS and CPI certifications.
