The Case Manager provides therapeutic behavioral services, service planning, and care coordination for clients with mental health or developmental needs. They are responsible for developing treatment plans, facilitating crisis prevention, and maintaining positive relationships with outside contacts.
Candidates must be at least 18 years old with a high school diploma and three years of relevant experience, or a Bachelor's/Master's degree in a related field. A valid driver's license and reliable transportation are required for travel within the assigned service area.
