Key Responsibilities
The Advanced Practice Clinician will conduct comprehensive patient assessments, including physical exams and medication reconciliation, within senior care settings. They will also collaborate with interdisciplinary teams to provide advanced illness planning and support for patients in sub-acute rehabilitation.
Requirements
Candidates must hold an active ANCC/AANP certification for NPs or NCCPA certification for PAs, along with an unrestricted state license and DEA authority. At least one year of clinical experience in a long-term care or geriatric setting is preferred.
