Key Responsibilities
The RN Case Manager evaluates and admits qualifying hospice patients while administering medications and treatments according to established guidelines. They are responsible for developing and implementing written plans of care and maintaining effective communication with patients, families, and physicians.
Requirements
Candidates must be graduates of an accredited school of nursing with an ADN-RN or BSN degree. A valid state driver's license and current automobile liability insurance are required, with at least one year of nursing experience preferred.
