What You’LL Be Doing
We're hiring Family Nurse Practitioners or Physician Assistants to serve as the primary clinician to provide support, treatment and management to patients. This is a full-scope, hands-on primary care role: you'll manage chronic disease, perform in-office procedures, and build long-term relationships with patients rather than cycling through high-volume, short visits.
Key Responsibilities
Provide longitudinal primary care across chronic disease management, preventive care, and acute visits to members.
Perform in-office procedures including I&Ds, suturing and suture removal, skin biopsies, and men's and women's health exams
Conduct visits in person, by phone, and by secure text/video, with typical visit lengths of 30 minutes to 1 hour depending on patient need
Diagnose and manage chronic conditions (e. g., diabetes, hypertension, hyperlipidemia) with the panel size and visit time needed to intervene before conditions progress
- Coordinate daily patient care and task delegation with an assigned Medical Assistant
- Document all visits and maintain patient records in the EHR.
Maintain 24/7/365 on-call access for patient communication via a company-provided secure line (Twilio-based; personal phone number is never shared with patients)
Work under a collaborating/delegating physician in accordance with Texas Medical Board supervision requirements
Participate in weekly team meetings and 1:1s and other clinical meetings as requested or required.
Required Qualifications
Active, unencumbered Texas licensure as a PA (NCCPA-certified) OR APRN-FNP (national board certified)
Minimum 2 years of post-graduate experience as a Family Nurse Practitioner or Physician Assistant in family practice or primary care
Demonstrated experience managing patients across the lifespan (pediatric through geriatric)
Hands-on experience performing I&Ds, suturing/suture removal, skin biopsies, IV placement, and men's/women's health exams
- Bilingual (English/Spanish), preferred
- Active, unrestricted DEA registration, or ability to obtain prior to start
- Current BLS certification, or ability to obtain within 30 days of employment
- Proficiency with electronic medical records and scheduling software
Preferred Qualifications
- Prior experience in direct primary care, concierge medicine, or an employer-sponsored clinic model
- Experience owning or managing a patient panel with full clinical autonomy
- Multistate licensure or telehealth experience
- Comfort with a lower-volume, higher-touch practice model versus high-throughput fee-for-service

Requirements
Candidates must hold an active, unencumbered Texas license as a PA or APRN-FNP with at least two years of post-graduate experience in family or primary care. Proficiency in performing clinical procedures and managing patients across the lifespan is required, along with current BLS certification and DEA registration.
