Key Responsibilities
The RN Case Manager coordinates and facilitates quality, cost-effective care for ESRD and CKD patients by collaborating with healthcare teams and providers. Requirements
Candidates must have an A.D.N. degree (B.S.N. preferred) and a current RN license with at least five years of clinical nursing experience. Proficiency in case management, basic computer skills, and a valid driver's license are required, along with the ability to work in a hybrid, field-based environment.
