Key Responsibilities
The RN Case Manager is responsible for processing palliative care referrals, coordinating patient care goals, and conducting home or facility visits. They must also ensure accurate documentation, manage billing reports, and serve as a liaison between medical directors and the palliative care team.
Requirements
Candidates must hold an active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license and possess at least three years of nursing experience. Experience in hospice, palliative, or home health care is preferred, and CHPN certification is considered a plus.
