Key Responsibilities
The Medical Social Worker performs psychosocial, emotional, and bereavement assessments to develop and implement goal-directed care plans for hospice patients. They also provide crisis intervention, individual or family counseling, and act as a care advocate while maintaining contact with community agencies.
Requirements
Candidates must hold an MSW degree from a CSWE-accredited school of social work. One year of experience in home health care and hospice experience are preferred, along with a valid driver's license and reliable transportation.
