Key Responsibilities

The pharmacist will conduct post-discharge medication reconciliation and provide remote consultations to optimize medication therapy for patients transitioning from hospital to home. They will collaborate with multidisciplinary teams to improve patient outcomes and reduce readmission risks through education and therapy recommendations.

Requirements

Candidates must hold a Pharm D degree and be licensed to practice pharmacy in Massachusetts, with board certification or PGY1 residency required. A minimum of 3 years of clinical pharmacy experience, preferably in an ambulatory setting, is also required.
