Key Responsibilities
The Nurse Navigator serves as the primary clinical contact to guide patients through specialty pharmacy care and coordinate with providers to ensure timely, connected services. They are responsible for educating patients, managing program referrals, and documenting interactions to improve patient outcomes and program quality.
Requirements
Candidates must hold a current, unencumbered Compact Registered Nurse license and possess at least 5 years of clinical experience in relevant settings. Proficiency in Windows-based software and strong organizational and communication skills are required for this independent, fast-paced role.
