Nurse Navigator - Family Planning Clinic

Key Responsibilities

The Nurse Navigator provides proactive case coordination and patient navigation to ensure optimal patient outcomes and continuity of care. They assess patient needs, manage utilization, and facilitate resources across the continuum from pre-registration through discharge.

Requirements

Candidates must be a graduate of an approved nursing school with a current New York RN license and at least five years of healthcare experience, including two years in an acute care setting. A BSN is preferred, and prior experience in women's health is strongly desired.
