Key Responsibilities
The RN Case Manager performs care management for members with complex and chronic needs by assessing, developing, and monitoring individualized care plans. This role involves coordinating care telephonically and in-person at various sites to ensure appropriate access to health services.
Requirements
Candidates must hold a current, unrestricted RN license in the state of Kansas and possess at least 3 years of clinical experience. A bachelor's degree in a health-related field is required, or an equivalent combination of education and experience.
