Key Responsibilities
The Advanced Care Partner collaborates closely with PCPs to share care and continuity management for complex patient panels, focusing on maintaining task category Service Level Agreements (SLAs) and providing real-time responses to patient messages. Responsibilities include clinical decision-making, owning medication adjustments for chronic conditions, completing necessary clinical paperwork, and ensuring continuity of care longitudinally.
Requirements
Candidates must have 5+ years of experience as an NP/PA, deep proficiency in remote/virtual medical care, and expertise in chronic disease management and high-risk caseloads, requiring current licensure in California. Strong self-starting discipline, comfort with technology, and excellent collaborative communication skills are essential for success in this team-based model.
