What will you do as a Hospice RN Case Manager? Here are just a few of the responsibilities:
Use clinical nursing knowledge, physical assessment, and procedural skills to deliver high quality patient care in the patient’s place of residence.
Develop and oversee plan of care under the direction of the physician and ensure appropriate interdisciplinary involvement to ensure the best outcome for each patient.
Perform initial and comprehensive assessments on all patients in accordance with policies and procedures.
Continuous visits and interdisciplinary assessment of the patient.
Deliver quality care and completes clear, concise, and accurate clinical documentation.
Provide patient, caregiver and family counseling and education as indicated in the plan of care.
Develop and continually update plan of care that identifies the care and services required to meet patient-specific needs.
What it takes to be an RN Case Manager with Care Initiatives Hospice:
Active Iowa RN license.
Active CPR certification.
Hospice or palliative care experience.
Two (2) years of verifiable experience working as a nurse (RN/LPN), preferred.
Current driver's license, automobile liability insurance, and the ability to travel between multiple locations within your region.
Home access to the internet.
Ability to obtain Hospice and Palliative Registered Nurse Certification (CHPN) within 24 months of employment.
