The Social Worker will conduct comprehensive psychosocial assessments and collaborate with interdisciplinary teams to develop individualized care plans for residents. They will also provide ongoing counseling, crisis intervention, and advocacy to ensure the holistic well-being and quality of life of residents.
Candidates must hold a Master’s degree in social work and possess current licensure or certification in the state of Connecticut. Previous experience in long-term care or geriatrics is required, along with strong assessment and interpersonal skills.
