Join Riverside Home Health & Hospice as a Hospice Social Worker (MSW/LCSW) – Monday-Friday in Grants Pass, Oregon.

Key Responsibilities
The Hospice Social Worker will conduct psychosocial assessments and develop care plans to support patients and families during end-of-life care. They will also collaborate with an interdisciplinary team to provide emotional support, education on advance directives, and coordination with community resources.
Requirements
Candidates must hold a Master's degree in Social Work (MSW) and possess an active Licensed Clinical Social Worker (LCSW) license in Oregon. Additionally, the role requires a valid driver's license, reliable transportation, and current BLS certification.
