• Conducting psychosocial assessments in patients’ homes
• Developing and implementing care plans
• Supporting patients and families with emotional and practical needs

• Master’s degree in Social Work (MSW)
• Licensed Clinical Social Worker (LCSW) in Oregon
• 2+ years healthcare experience preferred (hospice or home health a plus)
• Current BLS certification
• Strong communication and organizational skills
• Valid driver’s license and reliable transportation This role includes travel throughout Jackson County and surrounding areas, providing care in homes and facilities.
