Join Riverside Home Health & Hospice and make a difference in the lives of patients throughout our community. We are seeking a compassionate and motivated Nurse Practitioner to provide high-quality mobile and community-based healthcare services.

What You'll Do

• Diagnose and treat acute and chronic conditions
• Provide care in homes, Assisted Living Facilities (ALF), and Skilled Nursing Facilities (SNF)
• Conduct patient assessments, develop treatment plans, and prescribe medications
• Utilize telehealth and in-person visits
• Collaborate with patients, families, and healthcare partners
• Maintain accurate and timely clinical documentation

Qualifications

• Master's Degree from an accredited NP program
• Current state licensure and prescribing authority
• Valid driver's license and reliable transportation
• Strong communication and clinical decision-making skills
• Knowledge of HIPAA and healthcare regulations
