Key Responsibilities
The physician will provide direct patient care to a senior-focused panel, addressing both physical health and social determinants of health. Responsibilities include conducting wellness visits, managing chronic conditions, and performing screenings in an onsite clinical setting.
Requirements
Candidates must hold a Doctor of Medicine or Osteopathic Medicine degree and be board certified or eligible in Geriatric, Family, or Internal Medicine. A valid, unrestricted state medical license and DEA license are required for this position.
